Workflow Redirect Reports

We frequently have users redirect/delegate tasks to other users, as well as items that are worked through OOF (Out of Office Forwarding). Is there a way for me to extract or report on that information?

For example, we have specific approvals that are reviewed by examiners. When they look at the task data, they only see that Person #2 approved it. They cannot see that it was first assigned to Person #1, or when/why it was reassigned. We'd like to be able to prove that to them in reports.
