I thought I was agreeing to the full form for which i had not seen or read yet. It was confusing, and i was reluctant to sign. The issue was that I skimmed the copy and did not realize the check box agreement was only for the digital signature, not the form. If you added a clear header that "Terms and Services were for Digital Signature that could help as well as update the copy next to the check box to specify that the terms they are agreeing to are for their digital signature. It would be helpful to start with a quick overview of the process so users know what to expect. I was hesitant to give my name and contact information because I was unclear if this was part of the signature process, could you add copy or header - verify your identity.
