OR IN CONDITION IN THE DATA LOOKUPS IN THE FORM

I am looking for help for the Nintex Cloud form.
In the form “Data lookup option”, as of now we have if all or any conditions to be true to reflect the data.

But my requirement would be like to have or/in clause for certain column only and have the corresponding data reflected.
For example (connected to SQL server DB), in the below screenshot, I need 1,2,3 to be connected by “And” clause where as Status can be either approved or scheduled.

With the current scenario, the data will not be populated as it treats as it connected with And for all conditions listed above.
