Have an option to have the description display under the title of a field on a form

Having the description text be locked to being under the field the user fills out makes things confusing at times. Customers want to be able to move the description of the field to under the Title for better readability.

I have attached an image of how it should possibly look with the top one showing how it currently looks and the bottom the look we are wanting.

This was achieved using a label for the "Title" and aligning the text to the bottom of it and essentially making the "Title" of the field the description. This is not ideal method when multiple questions need to look like this.
