Multiple listitems from one submission form

Hello, i do have a list in which we can request one site at a time. However i would like to implement that i can create multiple separate listitems in one list from one form (request). So when i click "another row" i would like to create a new item in the list with an unique id. I did already see the solution for Nintex cloud but not yet for on-premises, because that doesn't contain the same options.
